Dear Patient,

Thank you for making an appointment with Eye Associates of Pinellas.  We want to welcome you to our practice and go over some information with you.

Please be sure to bring and/or wear your glasses or contact lenses at the time of your visit.  Also, it is very important you bring a list of your medications, including over the counter medications such as vitamins.

If you are on an HMO or other insurance that requires a referral, please be sure we have the referral at least two days prior to your scheduled appointment.  We are not able to see you without a referral.

We will call you the day before your appointment to confirm.  If you are unable to keep your appointment, please call in advance and we will gladly reschedule you.

Please print and fill out the Patient Registration form and Medical History form located on the left side of this screen.  Bring these forms with you to your first appointment.

Please be sure you arrive on time - if you are more than 5 minutes late for your first appointment we must reschedule you.  We will need a copy of your insurance card and a picture ID.  If you have a co-payment or a deductible, it is due at the time of service.
